This course will propose and teach a framework for the understanding and evaluation of chronic pelvic pain patients from a bio-psycho-social model that looks beyond the local tissue complaints and evaluates central nervous system sensitivity in chronic pain.

The Puzzle of Pelvic Pain: Pudendal Neuralgia or Central Sensitization?

Persistent Pelvic Pain is associated with physiological, psychological and social challenges. This course will propose and teach a framework for the understanding and evaluation of chronic pelvic pain patients from a bio-psycho-social model that looks beyond the local tissue complaints and evaluates central nervous system sensitivity in chronic pain. The treatment approaches taught in this course are designed to address both the local tissue needs, and central sensitization, with patient education in current pain science, graded imagery and neurodynamics. The focus of treatment for tissue dysfunction in this course will touch on myofascial tissue problems, but will focus on assessment and treatment issues of the Pudendal Nerve, as it fits in the larger context of the sensitized nervous system, including central nervous system changes.

This is a lecture-format and lab-based course. It is OPEN to non-pelvic health physiotherapists who want to learn more about persistent pelvic pain and central sensitization. To participate in the third day of the course, the participants are required to have previous experience in both rectal and vaginal manual palpation, as well as a solid basis of manual therapy skills for the treatment of pelvic floor dysfunction.

Course Instructors:

Carolyn Vandyken: Carolyn graduated from McMaster University in 1986, and has practiced in a variety of settings primarily focusing on orthopaedics. Her clinical focus changed to Pelvic Health and Incontinence in 2001. Carolyn is the co-owner of Pelvic Health Solutions, a company dedicated to the training of physiotherapists in the many facets of pelvic floor rehabilitation. Carolyn’s areas of clinical interest are female pelvic pain, pudendal neuralgia, and chronic prostatitis, with a focus on the central nervous system’s role in chronic pelvic pain.

Sandy Hilton: Sandy graduated from Pacific University (Oregon) in 1988 with a Master of Science in Physical Therapy. She has worked in multiple settings across the US with neurologic and orthopaedic emphasis combining these with a focus in pelvic rehabilitation for pain and dysfunction since 1995. Sandy has taught classes for Health Professionals and Community Education in returning to function following back and pelvic pain, assisted with Myofascial Release education and co-taught Advanced Level Male Pelvic Floor Evaluation and Treatment. Sandy’s clinical interest is with chronic and pelvic pain disorders for men and women.

Course Objectives:

To review pelvic floor anatomy, particularly with regards to the Pudendal Nerve

To understand the definitions of pudendal neuralgia, pudendal neuropathy and pudendal nerve entrapment

To understand the common impairments, functional limitations and disabilities associated with patients with pudendal neuralgia

To understand the NeuroMatrix Theory

To understand the Central Nervous system changes that occur in Persistent Pain

To understand the importance of Neurophysiology based pain education and adapt this to the pelvic floor

To understand the importance of downregulating the central nervous system in managing persistent pain problems, specifically pelvic pain

To understand the mechanisms of assessing pudendal nerve dysfunction in the pelvic floor

To understand the neurodynamics of treating pudendal nerve dysfunction in the pelvic floor

To understand the theory of graded motor imagery, and apply this to the pelvic floor

To understand the physiotherapy treatment techniques for Pudendal neuralgia

To recognize the importance of a multidisciplinary approach
